It is not known whether the immunogenicity of capsular polysaccharide (polyribosyl ribitol phosphate [PRP]) is related to the particle size, chemical structure, or physical form in which it is presented. The purpose of this study was to compare the ability of various forms of PRP to evoke protective antibody responses in infants and children. This was a randomized, double-blind, multicenter, placebo-controlled clinical trial of three vaccines containing chemically and physically modified PRP in different particle sizes in four countries. Approximately 5 mg of PRP in each dose was encapsulated into either microparticles (0.15-microns diameter) or multiparticles (4.1-microns diameter). In an injectable dose, the ratio of the amounts of encapsulated PRP in the microparticles and multiparticles was 1:10. In a second higher dose, the amounts of encapsulated PRP in the microparticles and multiparticles were the same. Both vaccines induced comparable and high responses of anti-PRP antibody in children. Immunogenicity was significantly higher in infants than in children. In this study, PRP in microparticles or multiparticles was highly immunogenic in infants and young children and resulted in a significant number of subjects with serologic evidence of protection.Article content
